CLMA 2019 Knowledge Lab - Grapevine, Texas

Reducing Lost Specimens One Sample at a Time: Our Journey

Track: Quality Improvement

Session Number: 802

Date: Tue, Apr 2nd, 2019

Time: 3:15 PM - 4:15 PM

Room: Grapevine 1-2

Description:

Laboratories continually assess the quality of results. Not as much effort has been focused on the non-analytic steps of the testing process, where most errors occur. In the fall of 2017, a team of M Health laboratorians began their journey identifying the potential for significant quality improvement in error reduction using automation, process improvements, and engineering tools. This presentation details our journey of discovery, our findings, and painful culture change.

Session Type: Breakout Session

Body of Knowledge Domains: Quality Management for Patient Safety

Content Level: Essential

Learning Objective 1: Identify and learn from your lab processes to decrease lost or missing specimens

Learning Objective 2: Learn how to employ metrics to track lost specimens and near misses, while building a culture that supports a robust error tracking process by removing the fears of making mistakes and encouraging transparency.

Learning Objective 3: Understand how standardized processes, tools, and behaviors lessen the chance for lost specimens to create a network to share best practices and quality tools

With her colleagues, Marya Awker and Vicki Erickson, Kayla Fahey-Ahrndt wrote an article detailing their learnings about reducing lost specimens after realizing their organization had to overcome a culture of tolerance. The article details their journey to create quality systems and structures to allow for continuous improvement and higher patient satisfaction.
